3.本项目的特定资格要求：





合同包1(彩色超声诊断仪升级阴超探头心电图机采购项目)特定资格要求如下:





（1）具有独立承担民事责任能力的法人、非法人组织或自然人，提供合法有效的营业执照/事业单位法人证书/专业服务机构执业许可证/民办非企业单位登记证书等相关证明，自然人参与的提供其身份证明；

（2）财务状况报告：提供2019年至今任意一年的财务审计报告（成立时间至提交磋商文件截止时间不足年的可提供成立后任意时段的资产负债表）或开标前六个月内其基本账户银行出具的资信证明；

（3）社会保障资金缴纳证明：提供2021年1月至今已缴存至少三个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的单位应提供相关证明材料；

（4）税收缴纳证明：提供2021年1月至今已缴纳至少三个月的纳税证明或完税证明，成立时间至提交磋商文件截止时间不足年的可提供成立后任意时段的相关完税证明）依法免税的单位应提供相关证明材料；
